مقایسه عملکرد بیمارستان‌های سازمان تأمین اجتماعی با بکارگیری تکنیک تحلیل پوششی داده‌ها (DEA)

چکیده

هدف پژوهش حاضر تعیین سطح کارایی بیمارستانهای سازمان تأمین اجتماعی و امکان مقایسه کارایی آنها با توجه به متغیرهای محیطی بیمارستان و مشخصات مدیران (نوع فعالیت، رشته فعالیت، سطح تحصیلات،سن، جنسیت، رشته تحصیلی) میباشد. روش پژوهش، در این پژوهش کلیه بیمارستان تأمین اجتماعی کشور مورد بررسی قرار گرفته‌اند (70 بیمارستان). در تعریف متغیرها تعداد تخت فعال، تعداد پزشک متخصص، تعداد پزشک عمومی، تعداد پیراپزشک، تعداد سایر پزشکان، تعداد پرستار، تعداد بهیارو کمک بهیار و تعداد سایر پرسنل به عنوان متغیرهای ستانده و تعداد پذیرش سرپایی، تعداد پذیرش بستری ، تعداد عمل جراحی، تخت-روز اشغالی، متوسط اقامت بیمار و ضریب اشغال تخت به عنوان متغیرهای نهاده مشخص شده است. جهت اندازه‌گیری عملکرد بیمارستان‌ها از مدل تحلیل پوششی دادهها (DEA) و ضمن مشخص کردن بیمارستانهای ناکارا، با استفاده از مدل اندرسون-پیترسون بیمارستان‌های کارا نیز رتبه‌بندی شدند. نتایج نشان میدهد میانگین کارایی بیمارستانهای تأمین اجتماعی در سال 1394 برابر با 99/0 بوده به‌طوریکه 58 بیمارستان کارا (83%) و 12 بیمارستان ناکارا (17%) هستند. ض. بنابراین بیمارستانهای مذکور با بهینه سازی تعداد مازادهای خود، حتی المقدور از مقادیر اولیه نهاده ها کم کنند تا به کارایی مطلوب دست یابند. بیمارستانهای مرجع برای هر بیمارستان ناکارا مشخص شد. سایر نتایج نشان داد که متغیرهای محیطی بیمارستان و مشخصات مدیران (نوع فعالیت، رشته فعالیت، سطح تحصیلات، سن، جنسیت، رشته تحصیلی) بر کارایی تأثیرگذار نیست.

The purpose of this study was to determine the level of efficiency of social care organization hospitals and to compare their efficiency with regard to the environmental variables of the hospital and the characteristics of the managers (type of activity, field of activity, level of education, age, gender, field of study). In this research, all social care hospitals in the country have been studied (70 hospitals). In the definition of variables, the number of active beds, the number of physicians, the number of general practitioners, the number of paramedical staff, the number of other doctors, the number of nurses, the number of volunteers and the number of other personnel as output variables and the number of admissions, admissions, Occupied bed, average patient accommodation and bed occupancy rate are identified as input variables. In order to measure the performance of hospitals, the DEA model was used to identify the inefficient hospitals, using Anderson-Peterson model hospitals.The results show that the average efficiency of social care hospitals in 2013 is 0.99, in which 58 hospitals (83%) and 12 inefficient hospitals (17%).In addition, in this study, the amount of inputs and outputs was determined for inefficient hospitals to reach the border.Therefore, the hospitals will optimize the number of their surpluses, even lower the initial inputs to achieve optimal performance. Other results also indicated that the environmental variables of the hospital and the characteristics of the managers (type of activity, field of activity, level of education, age, gender, field of study) did not affect performance.
